Register as a new patient at Wells Health Centre

To Register as a patient with us, we will need you to complete the online registration documents below. 

If you have any prescribed medication, please contact your existing GP practice for a supply if you will require these within 4 weeks of registering with us.

You do not need to show ID, proof of address or immigration status to register as a patient. 

Please check your answers carefully and ensure you have completed all the questions. Missing information or incorrect information slows down the registration process as our staff will need to contact you to gain the correct details. 

Once registered with us, you can manage your medical records online with www.nhs.uk/nhs-app

Alternatively, our reception team can set you up a more limited online account.  Please speak to them in person, with some photo ID.

You can only Register with us if you live within the Wells Health Centre Boundary.

Registering online as a non-English speaker

You can find guidance and advice on how to become a new patient if you’re first language is not English, or if you’re an asylum seeker. You can find out more information about the NHS and how to register and access emergency services on the NHS website.

Registering as a temporary patient

If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from a local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
